Clinical Observation of the Complications of CAPD Patients

Abstract
CAPD has established itself as an effective therapeutic modality as hemodialysis or renal transplantation in the treatment of patients with end stage renal disease. Although much progess of technology of CAPD has been made, the complications of CAPD have been an important problem. Retrospectively, we observed the complications of 178 patients with end stage renal diseases on CAPD at Chonnam University hospital from July 1988 to March 1993. @ES The results were as follows;
@EN 1. Patient ages ranged from 18 to 73 years and mean age was 43¡¾13 years. Male to female ratio was 116:62.
2. Underlying disorders of end stage renal diseases consisted of mainly chronic glomerulonephritis (28.8%), followed by diabetic nephropathy (19.0%) and hypertensive nephrosclerosis(7.1%). 3. The incidence of peritonitis was 0.71 per patient year and peritonitis free interval was 7.3¡¾0.6 months. The isolation rate of microorganisms from patients with peritonitis was 33.2% and the order of frequency of isolated organisms was S. epidermidis, S. aureus, Candida species, seudomonas species and E. coli. There were no obvious differences in initial leukocyte count of drained dialysate, duration of antibiotic treatment among various organisms. The results of peritonitis were treatment with only antibiotics (85.2%), catheter removal (12.1%), and death(3.8%). 4. The complications other than peritonitis were exit site infection or tunnel infection, hernia, hydrothorax, ultrafiltration failure and others. 5. Of 184 catheters, 38 catheters were removed and the peritonitis was the most common cause of catheter removal. The catheter survival rate was 88%, 75%, 64% and 47% at one year, two years, three years and four years, respectively. Mean survival times of all catheters were 33.6¡¾1.2 months.
6. The causes of death were most frequently cardiac disorders (38.5%), followed by cerebrovascular accidents (23%), and infections (15.4%). The overall survival rate in CAPD patients was 94% for one year.
